Kozak was taken to Norwood Hospital
after a two-car crash on Route 140 on Monday afternoon. The collision took
place at about 4:50 pm at the intersection of Route 140 and South Cross Street

Kozak was transported to the hospital
after he was removed from the Nissan by emergency personnel using the Jaws of
